STEP 3 - UPLOAD YOUR COMPLETED FORMS AND SUPPORTING DOCUMENTS TO SPORTS CONNECT PRIOR TO AUGUST 1ST.
HOW TO UPLOAD DOCUMENTS: When logged into the website navigate to "My Account", click the pencil icon next to your athletes name. You can then edit personal information, current registrations and upload documents.
2023 Pop Warner Medical Consent Form (Section 2) must be filled in completely and SIGNED by your child's DOCTOR. Because of insurance liability issues, NO CHILD WILL BE ALLOWED TO PARTICIPATE OR PLAY ON THE FIELD OR CHEER MATS WITHOUT A COMPLETED MEDICAL CONSENT FORM on file with the organization. We must have this form on file prior to the first day of practice.
Upload a copy of your child's birth certificate to Sports Connect. An original or photocopy of the birth certificate may need to be presented during book certification in August. Please have it available. This is extremely important if your child's team makes it to postseason competition in terms of eligibility.
Upload your child's 2022-2023 report card For ALL academic grading periods during the 2022-2023 school year. For kindergartners, proof of satisfactory participation. This copy is also extremely important if your child's team makes it to post-season competition.
